We are offering an in office membership plan that we hope gives more opportunity for healthy smiles. We want to simplify the process of treatment planning and billing for your dental work.

CHILD (4-13)$300
YOUNG CHILD (0-3)$100
Annual membership fee to be paid in full at initial visit and includes:
2 exams (comprehensive and periodic or 2 periodic)
Any necessary X-rays
2 routine cleanings *patients with periodontal disease will have an additional charge
2 fluoride applications (18 yo and under)
1 limited/emergency exam including X-rays
12% off any dental treatment- no maximum, no deductible, no waiting period
Crowns: flat rate- no additional charges for build ups and or pulp caps

This is not an insurance plan. This membership is only valid at the office of Anna Shirley, DDS
This membership cannot be used as a supplement to dental insurance.
12% discount on any additional treatment must be paid in full on day of service by cash, check or card only. Care credit not accepted with this plan.
Membership is non transferable and cannot be split with another patient.
Membership runs for 365 days from the date of purchase. No refund is offered if services are not used within 365 days.
No discounts rendered at specialty clinics. Referrals to specialists will be at the doctor’s discretion.
Services not offered by Dr. Anna Shirley: root canals, orthodontics, complicated extractions, periodontal surgery. These services will be referred to a local specialist and will not be discounted.
48- hour notice is REQUIRED to make changes to appointments. We understand that life happens, but we will give you multiple reminders for your appointment. Failure to give advance notice of appointment changes or no showing an appointment will require a $75 deposit in order to be rescheduled. This deposit can go towards dental services if you fulfill your appointment, otherwise it is non-refundable. We are a very small office and only schedule one patient at a time. We value your time and ask that you please do the same with ours!!
Current patients are eligible for these plans.
If you have come in recently and are not due for a check up/cleaning, we will not back date the discount on recent treatment, but we will honor the discount until your next cleaning if you join. The 365 days will start at the cleaning date.
Why would we offer this?
I want to establish a program that encourages comprehensive dental care with a regular recall program.
I am not “marking it up just to mark it down”. I took my fees that I started with when my practice opened and applied this “membership” to them.
Covid-19 has upped the price of almost every product we use but I also realize it has cut the income of almost every one of my patients. I have been wanting to offer something like this for a long time and this felt like the best time.
